5. EACH WEEKDAY BETWEEN APRIL 14TH AND MAY 30th 2008 AT APPROXIMATELY 7:10AM, B103 WILL ANNOUNCE THE “SONG OF THE DAY” AND WILL ANNOUNCE A TWO-HOUR WINDOW WHEN THE “SONG OF THE DAY” WILL BE PLAYED DURING THE SAME DAY.
6. IN ORDER TO WIN, THE LISTENER MUST BE THE 10th CALLER AT 815-874-2103 AFTER HEARING THE “SONG OF THE DAY” BEING PLAYED DURING THE ANNOUNCED TWO-HOUR WINDOW, WHICH WAS ANNOUNCED THE SAME MORNING AT APPROXIMATELY 7:10 AM.
7. PRIZES MAY NOT BE SUBSTITUTED OR TRANSFERRED TO ANOTHER PERSON.
8. FOR ANY PRIZE VALUED IN EXCESS OF $100, THE WINNER AGREES AS A CONDITION OF PARTICIPATION TO PROVIDE THEIR TAXPAYER IDENTIFICATION NUMBER OR SOCIAL SECURITY NUMBER PRIOR TO BEING AWARDED SAID PRIZE. THE WINNER IS RESPONSIBLE FOR ANY AND ALL TAXES ASSOCIATED WITH THE PRIZE PACKAGE AND WILL RECEIVE AN I.R.S. FORM-1099 FOR TAX REPORTING PURPOSES.
9. AS A CONDITION OF PARTICIPATION, PARTICIPANTS AGREE TO BE BOUND BY THE SOLE AND FINAL DECISION(S) OF THE CONTEST JUDGE(S) APPOINTED BY MAVERICK MEDIA OF ROCKFORD. ALL DECISIONS ARE FINAL.
10. PRIZE WINNER(S) WILL FORFEIT ALL PRIZES IF FOUND TO BE IN VIOLATION OF THESE OR OTHER SPECIFIED RULES.
11. ALL PRIZES MUST BE CLAIMED WITHIN 30 BUSINESS DAYS.
12. BY ENTERING, THE WINNER(S) CONSENTS TO THE USE OF THEIR NAME, LIKENESS, PHOTOGRAPH, AND VOICE FOR ADVERTISING AND PROMOTIONAL PURPOSES IN CONNECTION WITH THE CONTEST.
13. THESE RULES ARE SUBJECT TO CHANGE WITHOUT ADVANCE NOTICE; HOWEVER, ANY CHANGES WILL BE ANNOUNCED ON B103.
